As humans its enviable to fail. What is important is that we learn from our failures and thereby strive to improve and not repeat the failure.
Failures are interesting in that they provide a learning opportunity. Thus, the team should after addressing the firefight analyze the cause of failure and share lessons learned so that the issue and such patterns can be avoided in future.
